ORDERING INFORMATION

SPECIAL ORDER PROGRAM
The products in this brochure are available through the General Services Administration's (GSA) Special Order Program (SOP). Under the SOP, you will continue the same requisition and billing relationship with GSA. GSA, however, will place orders with the vendor for direct delivery to your agency.

ACTIVITY ADDRESS CODE
An Activity Address code is required to process all FEDSTRIP/MILSTRIP orders. Most agencies have a single office or person responsible for this code and the associated fund codes. If you are unsure who this is, call (877) 472-3777 and select option #2. DSN and Autovon calls cannot be returned. You may also fax your request on agency letterhead to GSA-FCSI at (703) 605-9850 or (703) 605-2737 with your agency, location and telephone number.

SUPPLEMENTAL ADDRESS
Please be sure to include a supplemental address if the signal code (Block 51 of Standard Form 344) is anything other than "A". Please also verify that your supplemental address is accurate and includes a telephone number and/or point of contact if possible.

FEDSTRIP/MILSTRIP
Agencies may submit requisitions via the FEDSTRIP process (civilian agencies), MILSTRIP (military agencies), or GSA Advantage! (all agencies). See DOD 4140 for complete details. Please submit your FEDSTRIP/MILSTRIP documents via mail or fax to:

Please ensure that a contact person and a commercial telephone number accompany all orders to expedite any necessary clarification. The office cannot return DSN or Autovon calls.

CONTRACT PRICES
Prices in this catalog are valid until July 31, 2008 with a provision for 4 one-year extensions. Prices are subject to change after August 01, 2008.

DELIVERY CONDITIONS
Normal Delivery
Shall not exceed 90 days after receipt of order (ARO)

QUICK SHIP DELIVERY
Shall not exceed 30 days ARO.
How to Specify Quick Ship Delivery
Make sure the priority code on the requisition is 01, 02, 03, 04, 05, 06, 07 or 08. These priorities will insure you receive quick ship on those items on which it is offered.

INSTALLATION
To specify installation requirements, please use an AOE or AO5 document. On each delivery order, list a minimum of two contact points and telephone numbers for coordination and delivery.

INSIDE DELIVERY
If requested, ordering activities must specify inside delivery in the remarks/exception data of the requisition and use Document Identifier AOE (for domestic) or AO5 (for export) as applicable. The name and telephone number of the point of contact must be included on the requisition. The following inside delivery rates are applicable for all items specified in this catalog.
Weight of units to be delivered within Consignee's Premises Rate:
Up to 911 pounds $ 35.10 minimum charge
Over 912 pounds $ 3.85/cwt. ($3.85 per 100 pounds)

The inside delivery rates are in effect for delivery within a consignee's premises when the building and elevator will accommodate the items being delivered. The inside delivery rate will be negotiated between the contractor and ordering agency in those instances when the building and elevator within a consignee's premises will not accommodate the items. The building and room number is required on the order and order must contain the contract and a phone number for appointment.

WARRANTY
Furniture included in this catalog is warranted for 24 months from date of shipment both within and outside the continental United States. The warranty states that the items will be free from defects in material and workmanship and will conform with the specifications and all other requirements of the contract.
